New Patient Visit
We don’t like to keep our patients waiting, so as soon as you arrive, you will receive a friendly greeting from our new patient coordinator. Enjoy a cup of tea or bottle of water at check in, and if you have any questions, please ask. We’ll either file your completed registration forms or give you forms to complete. Plan to spend about an hour with us during a complimentary consultation.
After we update your records and take digital X-rays or Panorex images, Dr. Gage or Dr. Bloom will talk with you about your background and learn about your expectations. Once the doctor finishes his analysis, we will begin planning the treatment phase, one step at a time. If your mouth could benefit from multiple treatments, your doctor will work with you to decide on what you need now, versus what you need over the next year, so you can plan a practical treatment course. Together, you and your dentist will create a solution that addresses your goals and improves your dental health.

To save time, print and complete these forms before your visit. Bring them to your appointment, and we’ll expedite registration. Alternately, you can complete forms when you arrive.
Insurance and Financing
We don’t want financial issues to stop you from getting the treatment you need. Our practice will file claims for most dental insurance plans and will always work to maximize your benefits. For your convenience, we also accept payment in the form of cash, check, or credit card, including American Express, Discover, MasterCard, and Visa. Third party financing is available through CareCredit and Chase Health Advance for qualified applicants. If you have any questions, don’t hesitate to call us.

Emergencies
Dental emergencies aren’t convenient, so we’re here for our patients all of the time.
